core responsibilities

  • Complete thorough assessments and reassessments to identify strengths, needs, and goals for individuals with complex mental health needs.

  • Develop and revise individualized care plans in partnership with the person served and their support network.

  • Coordinate referrals and link individuals to appropriate mental health, medical, educational, housing, and social services.

  • Ensure coordination across systems and providers and advocate for services that align with the individual’s needs and preferences.

  • Communicate consistently with individuals, families, caregivers, service providers, and other stakeholders.

qualifications

  • Bachelor’s degree in a human services field and at least one year of full-time experience serving individuals with mental health needs is required.

  • Equivalent experience as a provider, consumer, or advocate of mental health services with demonstrable skills relevant to case management can be considered in place of employment experience. 

  • Knowledge of mental health systems, trauma-informed care, and community resources is strongly preferred.

  • Must possess strong communication, documentation, and organizational skills.

schedule & work environment

  • Hybrid role with both remote and in-person work, 30-40 hours per week.

  • Flexible hours with availability typically between 9:00 AM and 5:00 PM, Monday through Friday.

  • Travel within Park and Gallatin Counties required for in-person meetings.

  • Competitive hourly compensation, $ D.O.E.

Sage's Targeted Case Managers play a pivotal role in linking youth and adults to essential resources and community support. In a rural state where resources may be limited, individuals can face challenges accessing vital support systems. Through practical assistance and links to our local partnerships, our Targeted Case Managers support youth and adults in overcoming barriers to getting the support they need.

The Targeted Case Manager (TCM) provides care coordination and advocacy for youth and adults with complex mental health needs. This role involves comprehensive assessment, collaborative care planning, service linkage, and ongoing monitoring to help individuals achieve stability, access support, and engage meaningfully in their communities. The position is hybrid, with a mix of remote and in-person work in Park and Gallatin Counties, and offers flexible scheduling—generally within weekday hours of 9:00 AM to 5:00 PM.
